CHCPS Application for Admission
CHCPS requires parents to submit an Application for Admission form
for each child that is requesting enrollment in the program. This form contains information about the child,
parents' contact information, desired date of enrollment and requested playgroup.Child Information Form
CHCPS requires parents to submit a Child Information form
for each child that is enrolled in the program. This form contains information about the child's allergies, physical conditions,
fears, and other specific information that would be useful for on-duty parents.Medical Form
CHCPS requires parents to submit an Authorization for Child's Emergency Medical Treatment form
for each child that is enrolled in the program. This form contains information about the child's medical background
(alergies, physical conditions, etc.), prfeferred physician, health insurance provider and parents' contact
information. This form authorizes CHCPS to obtain emergency medical services for your child if he/she
becomes ill or is involved in an accident.

Weather Status
CHCPS follows the closing schedule for DC public schools for holidays, snow days and the like. Therefore, if DC public schools are cancelled or have a two-hour delay for inclement weather, then the playschool is closed.
