CHCPS requires parents to submit an Authorization for Child's Emergency Medical Treatment form for each child that is enrolled in the program. This form contains information about the child's medical background (alergies, physical conditions, etc.), prfeferred physician, health insurance provider and parents' contact information. This form authorizes CHCPS to obtain emergency medical services for your child if he/she becomes ill or is involved in an accident.
